Alliance for Better Health is a collaborative network of more than 2,000 providers and community-based organizations who are partnering to transform how health care is delivered in our region, with a focus on Medicaid members and uninsured individuals.
We serve an estimated 193,000 Medicaid and uninsured members of our communities across a six county area in upstate New York (Albany, Fulton, Montgomery, Rensselaer, Schenectady, and Saratoga counties).
Alliance was formed in response to a New York State Department of Health initiative called the Delivery System Reform Incentive Payment Program (DSRIP). The shared goals of DSRIP and the Alliance are to achieve the so-called “Triple Aim” of improving patient care, improving the overall health and wellness of the communities we serve, and reducing health care costs. Additionally, we are specifically striving to reduce the number of avoidable hospitalizations and emergency department visits by 25% over a five year period.
Alliance is also known as a Performing Provider System (PPS) which means our partners are working together to develop and implement an integrated health care delivery system. We are seeking ways to improve how care is delivered. We are finding solutions to better coordinate care, break down barriers, fill gaps where health care services are needed, improve communications, lower costs, and become more efficient, all while ensuring that the patient is always at the center of his or her care.
Over a five year period, Alliance will be focused on eleven specific projects in order to achieve these main goals:
- Improve quality of patient care
- Improve the health and wellness of our communities
- Reduce health care costs
- Reduce unnecessary hospital and emergency department use by 25%
We are energized by the collaborative spirit that is driving the more than 2,000 providers and community organizations in our alliance.
This is challenging work, but we are dedicated to making positive changes to ensure the people we serve and the communities we live in are much healthier tomorrow because of our efforts today.
