Care Coordination Roles and Responsibilities: a list of various positions involved in care coordination.

Zone Sheets: Is a patient in the green zone (well), yellow (cautious) or red (emergency) for a particular condition? Zone sheets list symptoms and their corresponding appropriate level of concern and action required from a patient.

 

Medicaid Recipient Opt-Out Letter

Beginning in the fall of 2015, and extending through the spring of 2016, the Department of Health is mailing each Medicaid recipient a letter to introduce the DSRIP program, and to provide instructions for individuals to opt-out of sharing their data with the PPS. The following Opt-out letter FAQs is a resource for providers and provider organizations, should a patient come to them with questions about the process.

National Council for Behavioral Health Educational Video Series on Integrated Care

The National Council for Behavioral Health has created four short educational videos on implementing best practices for integrated care. These instructional videos demonstrate actions you can take to conduct motivational interviewing, warm hand-offs, morning huddles, and brief interventions within an integrated care setting.
